iCommit Transplant Australia App

Overview

In Australia, 2023 data shows mortality among kidney transplant recipients is partly linked to challenges in post-transplant management and the lack of a centralised, accessible resource to guide recovery and lifestyle practices. As transplant volumes rise, strengthening post-transplant support systems has become increasingly important.

A dedicated mobile application provides a vital solution by empowering patients to maintain medication adherence, monitor key health indicators, access reliable education, and manage follow-ups seamlessly. By complementing gold-standard care, it can improve long-term outcomes and enable patients to take a more active, informed role in recovery.

Goals

  • Support sustainable daily habits by simplifying routines around diet, activity, and health monitoring to reduce cognitive burden.

  • Enable early risk awareness by surfacing timely feedback and alerts that help patients recognise and act on potential issues sooner.

Problem statement

Patients struggle to maintain complex routines without continuous clinical support and a central portal to gather important post transplant education materials. 

Post transplant care centers are unable to track patient's progress as the datas are stored differently from each healthcare providers.

Product Handover

The product was handed over with a complete set of UX deliverables, including final UI designs, design system guidelines, interaction specifications, and annotated user flows to support smooth implementation.

 

Detailed documentation of key features, edge cases, and usability considerations was provided alongside success metrics and tracking recommendations to enable ongoing performance monitoring.

 

A walkthrough session was conducted with stakeholders and developers to align on functionality, design intent, and future scalability, ensuring the client is well-equipped to maintain, iterate, and optimise the product post-launch.
