In Australia, 2023 data shows mortality among kidney transplant recipients is partly linked to challenges in post-transplant management and the lack of a centralised, accessible resource to guide recovery and lifestyle practices. As transplant volumes rise, strengthening post-transplant support systems has become increasingly important.
A dedicated mobile application provides a vital solution by empowering patients to maintain medication adherence, monitor key health indicators, access reliable education, and manage follow-ups seamlessly. By complementing gold-standard care, it can improve long-term outcomes and enable patients to take a more active, informed role in recovery.

A walkthrough session was conducted with stakeholders and developers to align on functionality, design intent, and future scalability, ensuring the client is well-equipped to maintain, iterate, and optimise the product post-launch.
